USD116 SCHOOL-BASED HEALTH CENTER
USD116 families can schedule vaccinations and/or physicals at the Urbana School Health Center. USHC offers flu shots, physicals, vaccines, health exams, and other primary care needs. To schedule an appointment, call (217)-239-4220.
USHC is a partnership between Promise Healthcare and the Urbana School District. Promise Healthcare is a local community health center providing primary medical, behavioral health and dental care to patients regardless of ability to pay.
The Urbana School Health Center is located at 1002 S. Race Street, adjacent to Urbana High School.
The USHC is open Monday-Friday from 8 a.m – 5 p.m. For a medical or counseling appointment, please call (217) 239-4220. For a dental appointment, please call (217) 403-5477 and ask for the school dental clinic.
Services Provided
Medical Services: The Urbana Health center provides physical exams (school and sport physicals), immunizations, treatment for acute and chronic medical conditions (asthma, diabetes, ADHD, etc.), reproductive health services, treatment for sport-related injuries and sub-specialty referrals, and individual and family dietary management.
Dental Services: The Urbana Health Center provides dental services for children and adults, school dental exams and cleanings, and treatment for cavities and extractions on-site.
Counseling: the Urbana Health Center also offers individual and family counseling, school-based counseling, case management, and resource and referrals.
How to access the Health Center
Parental consent required (except for Adolescent Confidential Health Services as per State of Illinois Statues). See the forms in English and Spanish in the sidebar. French and Mandarin are available at the clinic. There are exceptions for Adolescent Confidential Health Services as per State of Illinois statutes. Parents MUST attend the first medical visit, but parents do not need to be present for medical visits for middle school and high school level students thereafter. Parents MUST also attend the first dental visit; parents of high school level students need not attend thereafter. Students or parents may call and make appointments. Same day appointments are available for medical and some counseling. Same day appointments are also available for school/sport physicals. Patients may need to visit Frances Nelson Health Center or come back for immunizations.
Method of Payment
The USHC accepts most insurance plans and an income based sliding fee scale is available for those who are uninsured. The USHC does not turn anyone away based on ability to pay.
