If you live in Urbana and your child will be 5 years old on or before September 1 of the current year, you can register your student for kindergarten.

How to register for Kindergarten:

If you missed the kindergarten registration event, please visit your assigned elementary school from 9-11 a.m. or 1-2:30 p.m. to get them registered. If you have questions, email kindergarten@usd116.org.

Please bring the following documents:

• Photo ID (driver’s license, State ID, passport)
• Child’s official birth certificate (has a raised seal)
• Proof that you live in Urbana
  • signed lease/mortgage OR TWO of the following listing the parent/guardian’s name and address:
    • driver’s license
    • utility bill
    • voter registration card
    • change of address form
• Your child’s physical examination form. 
• Your child’s eye examination form completed by a licensed ophthalmologist or optometrist
• Your child’s dental form. Dental exams are valid for 18 months
• Phone numbers for doctors and emergency contacts

If you do not submit hard copies of the required documents during registration, please email copies to: kindergarten@usd116.org

IMPORTANT: If you submit your forms in-person or online, you do not have to register in-person during Centralized Registration in July. Beginning Aug. 1, plan to go to your student’s school to pick up a handbook, pay fees or fill out a fee waiver form.
