Registration for NEW students will be held July 27 from 9 a.m. -1 p.m. and July 28 from 2-5 p.m. at Urbana Middle School. This is for new families only. This includes students enrolling in school for the first time, students returning after being out of the district, or for students transferring to USD116 from another school district.

• Proof of residency
• Student’s birth certificate
• Student personnel record – General Information
• Student transfer form: ONLY for students transferring from another IL school *NOTE* All Illinois public school districts are required to provide this form to any student who is moving out of their school district to let the new IL school district know whether or not the student is “in good standing” and whether or not their medical records are up-to-date.

Required Medical Information

• Physical form/ forma en Español
• Immunization records
• Dental exam – Grades K, 2, 6
• Eye exam
• Emergency Medical Authorization
• Health History Form

Proof Of Residency Requirements

Any adult who registers a child in the Urbana School District must provide:

• Photo ID (ONE of the following is acceptable):
  • Driver’s license
  • State ID card
  • Passport
  • Other Photo ID
