Through a partnership with Champaign-Urbana Public Health District (C-UPHD), Urbana School District is offering the opportunity for our families who received their first vaccine dose on November 5 to return on Thurs., December 2 from 3:45 – 6 p.m. and Fri., Dec.3 from 3:45 – 6 p.m at Urbana High School.  

You will need to complete a sign-up form for each person who plans to receive a vaccine (i.e., one form for each student). Parents/Guardians or immediate family members are required to be present with the student at the time of the vaccine. If your child(ren) has already received their 2nd dose, then, of course, there is no need for you to sign up for this clinic. Please sign up by Wednesday, December 1 at noon, if possible.

To register for your appointment, you will need to do the following:

• Complete the google sign-up form for your student(s) who will be participating
• After completing the form, follow the steps to the scheduler link
• Select a time slot and submit (remember you are not done until you click submit).
• Complete the Champaign-Urbana Public Health District’s consent form (see attached)
• Remember you need to PRINT the attached form and BRING it with you to your appointment.

A few helpful reminders for parents and guardians:

• You and your student(s) will be asked to identify yourselves when you check in at the check-in station.
• You and your student(s) will need to wear a face-covering/mask and practice social distancing while in UHS.
• You and your student(s) will be at UHS for at least 20-25 minutes. This includes at least 15 minutes for observation after they receive the vaccination.
• You will want them to wear clothing to allow easy access to the upper portion of the arm for the injection.
