USD #116 is offering a booster clinic for vaccinated staff and students on Thursday, February 17. 

The clinic will take place at Urbana High School from 3:30-6 p.m.  When you arrive, you should park in the UHS main parking lot and enter at Door 9/Gymnasium, accessed via Tiger Lane.

We hope that all eligible students 12 and over needing a booster will take advantage of this opportunity. 

The Federal Drug Administration (FDA) has authorized the Pfizer booster vaccine for anyone who has received two doses of the Pfizer or Moderna vaccine or the single-dose Johnson & Johnson vaccine at least 5 months earlier.

To register for your appointment, you will need to do the following:

• Complete this form for anyone who will be participating
• After completing the form, follow the steps to the scheduler link
• Select a time slot and submit (remember you are not done until you submit)
• Complete the Champaign-Urbana Public Health District’s consent form.
• Remember you need to PRINT the consent form and BRING it with you to your appointment
