NOTICE: Self-Administration of Asthma Medication Law Amended
En Español: ENMIENDA DE LA LEY DE AUTO-ADMINISTRACION DE MEDICAMENTO PARA EL ASMA
Illinois school code regarding the self-administration of asthma medication has been amended; see Public Act 096-1460, Sec. 22-30. Previously the law required that your child’s health care provider sign a written authorization, allowing your child to self-administer their reliever/rescue asthma inhaler medication while at school. The law has been changed, and now allows you to submit a current prescription label instead of the signed written authorization from your child’s health care provider. The submitted prescription label must contain the following information; the name of the medication, the prescribed dosage, and the time at which, or circumstances under which the medication is to be administered. If you do not have an appropriate prescription label, you may submit written authorization from your child’s healthcare provider.
As always, the parent/guardian must still complete and submit a Self-Administration of Asthma Medication Authorization Form. This form can be obtained at the USD 116 website, at your child’s school, or by contacting the District Nurse at 384-3616.

Vision and Hearing Screenings
Vision and hearing screenings will be administered during the school year according to Illinois State Public Health guidelines. The mandate requires hearing screenings for all students in preschool, kindergarten, 1st, 2nd and 3rd grade, and all students in special education classes. Vision screenings are required for all preschool, kindergarten, 2nd grade, 8th grade and all students in special education classes. The screenings are conducted by the district nurses.
Vision screening is not a substitute for a complete vision and eye evaluation by an eye doctor. Your child is not required to undergo this vision screening if an optometrist or ophthalmologist has completed and signed a report form indicating that an evaluation has been administered within the last 12 months.
In addition to these students any student entering Urbana School District who has not previously been screened and any student referred for a special education case study will be screened for both vision and hearing. Any student suspected by a parent, guardian or teacher of having a vision or hearing problem may also be referred for testing.

If you wish to exempt your child from the Vision and/or Hearing Screening please contact the District Nurse at 384-3616. If your child’s school is not listed, please check the site again as the list is updated regularly.
